unable to add attributte set based on default

Hello Please help.

I am having 2 issue.

1. I am unable to add a new attribute set based on default,
When I click the add button it takes a while and throw a time out error, after I refresh it shows that the set has been added but not based on default(without the default values)
I have increase my php time but I am still having the error.

2. I tried Re-indexing Product Flat Data but I get “Cannot initialize the indexer process”

Please help.
what do I do?
