contact page not displaying correctly

Hi,

I’m having some issue with the contact page on a site, it appears fine, but when you submit it changes and doesn’t look how it should.

the page is http://www.disposablemedicalinstruments.co.uk/contact
when you submit it goes here http://www.disposablemedicalinstruments.co.uk/contacts

Which doesn’t display how it should.
